Please read the following BEFORE submitting your registration form. If you do not agree with the terms and conditions below, do not submit your application. You must make a deposit the day of submitting your registration form. This can be done by using your credit card or by mailing a check for the correct deposit or full payment. Checks must be received within 7 days of the registration form or your registration will be canceled. Thank you
- I have read the camp rules, understand them and agree that my child shall abide by them. I also understand that if my child violates these rules he/she may be sent home with no refund.
- I understand that all information/communication from the camp will come via email.
- I understand that my child will not be permitted to check into camp until all camp fees have been paid in full.
- I understand that my deposit is non-refundable. No exceptions.
- I understand that if my child voluntarily leaves camp early, only the unused portion of the room and board fee is refundable.

A $200.00 deposit for the residential camp or $50.00 deposit for the day camp should be submitted to the camp office to reserve enrollment at camp. Please send your deposit to Rob Russo, Director, Score and Save Soccer, 2 Sunset Hill, Granville, OH 43023. Checks should be made out to Score and Save Soccer.
You may also pay your camp fees with a credit card.
